Tissue regeneration in pediatric distal finger trauma with semi-occlusive dressing: Case report.

Abstract

Introduction

Traumatic injuries of the distal phalanx account for approximately 4.8–9% of pediatric orthopedic emergency visits. Although surgical treatment has traditionally been used in cases with bone exposure, in recent years conservative management using semi-occlusive dressings has been shown to promote tissue regeneration and achieve satisfactory functional and aesthetic outcomes.

Case presentation

We report the case of a pediatric patient with a traumatic injury of the distal phalanx of the finger, associated with limited bone exposure and a distal fracture (Allen III / Ishikawa II). Management was performed using conservative treatment with a semi-occlusive dressing and periodic clinical follow-up. During follow-up, progressive regeneration of the fingertip pulp, complete epithelialization of the wound, and adequate functional recovery were observed, with no evidence of infection or other complications. Complete healing occurred in approximately 32 days.

Discussion

The favorable evolution observed in this case is consistent with current evidence highlighting the high regenerative capacity of distal tissues in children when a protected moist environment is maintained. Conservative treatment may help avoid unnecessary surgical procedures, reduce healthcare costs, and decrease the morbidity associated with more invasive interventions.

Conclusion

Conservative management with semi-occlusive dressings represents a safe and effective therapeutic alternative for distal fingertip injuries in selected pediatric patients, even in the presence of limited bone exposure, provided that appropriate clinical follow-up is ensured.
