Abstract

Objective: To analyze the association between wastewater discharge and the presence of antimicrobial resistance in enterobacteria in surface water bodies in Latin America. 

Methods: A systematic review of observational and microbiological studies was conducted using PubMed, Scopus, Web of Science, and LILACS. Studies conducted in Latin America that evaluated antimicrobial resistance in enterobacteria associated with wastewater discharge were included (2020-2026). 

Results: Narrative synthesis identified E. coli as the predominant microorganism across all water bodies, with multidrug-resistant (MDR) profiles ranging from 91% to 100%. The resistance genes blaCTX-MblaKPCblaNDM, and mcr-1 were detected in hospital effluents, wastewater treatment plants (WWTPs), and agricultural irrigation waters, confirming dissemination pathways into the food chain. Conventional treatment systems were found to be insufficient for the removal of clinically relevant antimicrobial resistance genes (ARGs). 

Conclusions: Wastewater in Latin America constitutes an active route for the dissemination of antimicrobial resistance, with critical ARGs detected even in matrices intended for human consumption. Environmental surveillance of ARGs should be incorporated into national epidemiological monitoring systems, while regulatory frameworks should be updated and investments in advanced sanitation technologies promoted under the One Health approach.
